Registered Nurse (Outpatient - Home & Community-Based Care)

Summary Home & Community Based Care (H&CBC), Hospital in Home Program (HIH) provides care for recently early discharged, stable Veterans requiring acute hospital-level care and services after discharge. The HIH program assists the patient/family/caregiver in developing appropriate plans to guide them through immediate medical changes that have occurred. Incumbent of this position delivers fundamental knowledge-based care to assigned clients while developing technical skills in the veteran's home. Responsibilities The nurse assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
