Job Description

  Job Title: Hospice Registered Nurse (RN)
 
Day 5x8-Hour (08:00 - 17:00)

  Position Summary:
 
This center is needing a Hospice RN with Homecare Homebase (HCHB) experience who can hit the ground running. The RN will be assigned a tablet, see patients the same day each week, and participate in the on-call rotation.

  Work Schedule:

 On-call rotation

 Monday   Friday (8:00 AM   5:00 PM)

  Position / Specialty: RN   Hospice

  Charting Requirements:

 Homecare Homebase (HCHB) & OASIS

 Must complete charting while at patient home

  Average Patient Load: 4 6 patients per day
