This person will work in both Harrison, MI and Gladwin, MI
SCHEDULE
This is a part-time position, 40 hours per 2 week pay period. The specific schedule will be discussed further during the interview process.
Compensation and Benefits:
Hourly Rate: $27.82-$36.55 depending on experience.

General Description : Provides health services, care management and care coordination to consumers through pre-evaluation, assessment, planning, education, intervention, and review. Participates as part of an inter-disciplinary team.
