The exciting opportunity

• Permanent full-time role working all shifts
• Deliver high quality clinical care to residents of our 61-bed Grafton aged care facility 

• Formulate, review and update care plans aligned to residents needs and wishes
• Conduct and analyse assessments to determine care needs
• Complete comprehensive documentation and progress notes that reflect residents’ care
• Provide supervision and direction to Personal Care Workers
• Collaborate with the multidisciplinary team to ensure holistic care
