Summary This Registered Nurse - Collaborative Care position is located in the Nursing Service in the Flow Coordination Center at the VA Boston Healthcare System, located at the West Roxbury campus location. This is a full-time 40 hours per week position. Responsibilities The Collaborative Care nurse is a registered nurse responsible for identifying and facilitating options and services for meeting patients' health needs while decreasing fragmentation and duplication of care and enhancing quality, cost-effective clinical outcomes. The Collaborative Care RN performs the dual role of inpatient discharge planning and utilization review with a mind towards identifying barriers to and delays in transitioning patients to the optimal level of care, as well as, arranging for post-acute home services.
