Summary The Clinic RN works in collaboration and continuous partnership with clinic patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources. The Clinic RN collaboratively assists MLHS patients applying evidence practice to achieve optimal care.

+ Perform Nurse visits* Facilitate process and work flows to augment quality initiatives (meaningful use, PCRS, MN community measures)*
+ Assist clinic staff and providers to enable efficient flow of patient care. Tasks to include but not limited to; rooming of patients, administering injections, facilitating patient requests by phone and patient portal and coordination of outside referrals*
