Summary The Case Management Registered Nurse (CM RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The Social Work and Nursing Case Management Service Line is a multidisciplinary team that addresses the healthcare needs of veterans. The incumbent will provide nursing support to inpatient medicine and specialty care teams, aiding in the coordination of post-discharge services.
