Day Shift, inpatient RN CM Acute hospital experience, Day shift - , Monday through Friday, every other weekend commitment; 40-hour work week. This is for Inpatient Case Management with focus on timely Discharge Planning.
We are comprised of a 515-bed hospital, two urgent cares, home care services, comprehensive cardiology care and a vast scope of services.
Job Summary:
Provides excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes.
