**Job Opportunity: Registered Nurse (NYIAP) - Brooklyn, NY**

We are seeking a passionate and experienced **Registered Nurse** to support the New York Independent Assessor Program (NYIAP). In this role, you will perform vital home assessments for vulnerable populations to determine eligibility for long-term community-based services while mentoring a team of Nurse Assessors.

**The Basics**

+ **Pay Rate:** $60/hr
+ **Location:** Brooklyn, NY (2 days work from home, 3 days in the field)
+ **Hours:** Mon–Fri (8am–6pm) | Sat–Sun (10am–6pm) _Schedule flexibility available. 37.5 hours weekly_

**Key Responsibilities**

+ **Clinical Assessments:** Conduct initial in-home evaluations (or telehealth) using the **UAS-NY tool** to determine health status and care needs.
