We have an exciting opportunity for a compassionate and motivated individual to join Inclusion Recovery Hampshire, a drug and alcohol treatment and support service.
In this role, you will manage your own caseload and deliver high-quality, person-centred care throughout each service user's treatment and recovery journey. This will include completing assessments, care planning, delivering a range of interventions, and supporting individuals to achieve their recovery goals from referral through to discharge.
You will work as part of a dedicated, multi-disciplinary team, alongside prescribers and other professionals, to provide health support including blood-borne virus (BBV) testing clinics. You will embrace a recovery-focused approach and ensure all records and documentation are accurate, timely, and up to date.
