Job Description

What you’ll do:

  • Build trusting, respectful relationships with people accessing CPS support
  • Develop and deliver individualised recovery plans focused on people’s strengths, goals, and needs
  • Support individuals through the NDIS access and planning process, including gathering evidence and attending meetings
  • Help people build their capacity for daily living, social interaction, education, and employment
  • Link individuals with other services, supports, and local opportunities
  • Collaborate with families, carers, and service providers to support holistic outcomes
  • Work up to a fixed 75-hour fortnight (Monday to Friday, business hours)
  • Use your own vehicle to travel between participants (mileage reimbursed). Most travel will be within 30 minutes of each location.
