General Overview: The Rapid Re-Housing Case Manager helps individuals and families move from homelessness to permanent housing with a focus on long-term stability. Responsibilities include assessments, creating housing plans, assisting with searches, completing HUD inspections, and coordinating move-ins. The role provides ongoing case management in budgeting, income, employment, and self-sufficiency, while managing rental and utility assistance.
