Primary Care Physician

Position Overview
We are seeking a board-certified or board-eligible Primary Care Physician who thrives in a value-based, patient-centered care environment . The ideal candidate will work closely with a dedicated interdisciplinary team-including an Advanced Practice Provider (APP), nurse care manager, behavioral health therapist, and clinical pharmacist-to deliver high-touch, coordinated care to a defined patient panel.
This is not a typical fee-for-service role. Our providers focus on long-term outcomes rather than visit volume. In this role, you will collaborate with other physicians, nurses, therapists, and pharmacists to deliver comprehensive care to patients enrolled in value-based care programs, such as Medicare Shared Savings Programs (MSSP). Your contributions will be pivotal in reducing the total cost of care, improving quality metrics, and ensuring equitable access.
