Primary Care Liaison Practitioner

The post holder will work as part of the Primary Care Networks (PCNs)Multi-Disciplinary Team across member practices with specific responsibilitiesto liaise with and support Primary Care Colleagues to ensure seamless care andassessment. Provide advanced triage process directly to GPs, directly liaisewith GPs and act as a single point of contact for the PCN where there isuncertainty about the level of mental health need that requires furtherexploration.

Act as aliaison point between primary and secondary care services, including liaisonwith Care Coordinator where the service users presenting in primary care isalready known to Navigo's community mental health team.

Workas an active member of the PCN MDT undertaking supervisor role
