Practice Pharmacist

We are looking for a motivated, patient centred Pharmacist to join our friendly, forward-thinking coastal practice.

In post, you'll play a key role in medicines optimisation, long-term condition management, and supporting safe, effective prescribing.

This role is ideal for someone who thrives in a collaborative environment, enjoys problem solving, and wants to make a meaningful impact on patient care within a supportive environment.

The Role includes:

Medication reviews - structured medication reviews (SMRs), polypharmacy reviews, and medicines reconciliation.

Long term condition support - contributing to management of hypertension, asthma, COPD, diabetes and more.

Clinical support - assisting GPs with complex prescribing queries and safety monitoring.

Quality improvement - contributing to audits, QOF, IIF and medicines safety initiatives.
