Physiotherapist (PT) - Community Based - Toronto and surrounding GTA area

The Care Company is seeking a skilled and dedicated Physiotherapist (PT) with strong home and community care experience to join our growing team.

This position requires in-person home visits with vulnerable community members.

Key Responsibilities

• Provide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progression
• Develop, implement, and adjust individualized treatment plans in alignment with client goals
• Educate clients and caregivers on home exercise programs, safety strategies, functional mobility, and equipment use
• Identify and address fall risks, environmental barriers, and safety issues in the home
• Collaborate with OTs, PSWs, nurses, physicians, and HCCSS/ care coordinators to support coordinated care
