Physical Therapist - Home Health - Dublin, OH
Join a home health team in Dublin, Ohio as a traveling Physical Therapist. Provide patient evaluations, develop and implement individualized treatment plans, and deliver therapeutic services according to physician orders. Supervise and evaluate assigned Physical Therapy Assistants and Home Health Aides. Conduct home environment evaluations, document patient progress, participate in discharge planning, and collaborate with interdisciplinary team members to ensure continuity of care.

Minimum Qualifications
- Successful completion of an accredited Physical Therapy program (Bachelor's, Master's, or Doctoral degree) approved by the appropriate accrediting body.
- Current, unencumbered Physical Therapist license for the state of assignment prior to start.
- Current CPR certification.
