**Description:**
This position evaluates and treats patients of all ages. Develop treatment plans with involvement in family/support systems to meet the individual’s functional goals. Provides direct supervision of support personal as related to patient care.
**What you will Do:**

+ Evaluates plans, directs, supervises and conducts habilitative and rehabilitative programs for patients with physical limitations.
+ Consults and communicates with patient, family, and other multi-disciplinary team members and service providers.
+ Records and reports patient’s progress towards established goals, and data related to care in the patient’s medical record/chart. Instructs patients in exercises, ambulation, and activities of daily living, safe body mechanics, home exercise programs, and return to work precautions.
