Patient Focused Care Responsibilities

• Deliver clinical services such as medication reviews, chronic disease management and care plan development. Administer vaccines and immunizations
• Determine appropriateness of drug therapy and monitor for effectiveness, side effects, and interactions
• Counsel patients on prescriptions, over‑the‑counter products, and health‑related topics, including indications, contraindication, side effects, and proper usage and storage
• Ensure compliance with all federal, provincial, and regulatory standards governing pharmacy practice
• Assist with daily pharmacy operations and perform other duties as assigned

Qualifications

• Licensed and in good standing with the Ontario College of Pharmacists
• Injection certification is required
• Collaborative mindset with the ability to work independently or in a team environment
