Personal Support Coordinator - Long Term Care

Your Opportunity:

The Elk Point Healthcare Centre has an opportunity for a passionate Personal Support Coordinator (PSC) dedicated to patient- and family-centered care. This role will be an integral part of the multi-disciplinary team in Long Term Care which is made of 30 beds. You will build meaningful relationships as you work with clients daily providing personal assistance and support services to those in your care. Superior interpersonal skills will be used daily as you work closely with clients, families, caregivers, and health professionals. You will help maintain a safe environment, observe and report on patients' health and well-being, and ensure effective communication. Your role includes direct client care demonstrating genuine interest and respect for clients and families in your care.
