Job Description

Responsibilities:
Provides support and/or assistance to the residents to meet their needs and expectations as per the residents’ individual plan of care including but not limited to personal hygiene, dressing, mobility, toileting, feeding. 
Provides support by maintaining the cleanliness and neatness of resident living area. 
 Ensures the resident’s equipment is safe and functional. 
Accurately measures weight, height, intake and output and documents in the electronic health record [e.g. Point of Care (POC)]. 
Communicates all changes in resident status to the Registered Practical Nurse or to the Registered Nurse on duty. 
Improves the residents’ optimum functional ability by providing restorative care processes in the specific personal care area using assistive and adaptive measures as ordered.
