PCN Clinical Pharmacist

This is an exciting opportunity for an experienced and motivated onsite clinicalpharmacist to join our growing, supportive team.

Please note this is an onsite role and not remote working.

You should be able to work independently and demonstrate a high standard of safe clinical decision making with a broad and relevant knowledge base.

The post holder will provide clinical leadership onmedicines optimisation and quality improvement and manage some aspects of theQuality and Outcomes Framework and Investment and Impact Fund where appropriate.

We are looking for a candidate who is a qualified Independent Prescriber.

Main duties of the job

Provide support to general practice staff with regards toprescription and medication queries and help support the repeat prescriptionsystem

Lead on the monitoring of practice prescribing against national guidelines and formulary recommendations,including High Risk Medication prescribing/monitoring
