PASA Case Manager

Role

We are seeking a dedicated and experienced professional to join us as PASA Case Manager . This role will report into the PASA Program Lead – Colorado. This is a Full-Time Hybrid position based in Denver, CO.

Key Requirements:

• 2-3 years of direct experience as a case manager or similar role specifically within Colorado’s PASA program (CES/SLS Waivers).
• Familiarity with PASA services such as Community Connector, Homemaker Enhanced, Respite, or Supported Community Connector.
• Experience in care coordination, patient onboarding, caseload management, and Medicaid home health compliance.
• Strong organizational skills with proficiency in Google Workspace, Salesforce, or equivalent tools for documentation and reporting.
• Excellent communication and empathy, with the ability to support diverse families in a person-centered approach.
