Meet psychosocial needs of patients and families through clinical assessment, crisis intervention, supportive counseling, case management, and case consultation with integrated health care team and collaboration with community professionals.

**Job Responsibilities:**

+ Assesses patient and family needs and coordinate, advocate and problem solve to enhance their functioning.

+ Completes psychosocial assessment, including the gathering of social, psychosocial, cultural and financial information to determine level of support the family requires.
+ Develops plan based on assessment and assists family in implementing plan (referrals, follow up calls, DCF reporting).
