The Outpatient Coordinator supports the Case Management team by coordinating non-clinical services for members who meet case management criteria. This role is responsible for scheduling services, verifying benefits, maintaining accurate data, and engaging members through outbound calls to introduce and support care coordination programs.
The ideal candidate is detail-oriented, customer-focused, and thrives in a fast-paced, team-driven healthcare environment.
Key Responsibilities

+ Coordinate non-clinical services including:

+ Medical appointments (primary care & specialists)

+ Home health services

+ Durable Medical Equipment (DME)

+ Transportation services


+ Conduct outbound calls to members to introduce and enroll them in Case Management programs

+ Verify eligibility and benefits; request and manage medical records
