Job Description

SUMMARY

This job will assist the patient, family and care giver to overcome healthcare barriers (including but not limited to social, financial, or physical barriers.) Responsible for conducting on-going assessments of need and facilitate referrals to additional Support Services throughout the Oncology continuum of care. Using the nursing process and evidence based knowledge, the Oncology Nurse Navigator provides education and resources to facilitate informed decision making (by the patient and/or family.) BENEFITS
