To provide Occupational Therapy services to patients to include evaluation, assessment, treatment program planning and implementation, and related documentation, and communication.ESSENTIAL FUNCTIONS:
Evaluates patients to obtain and interpret data necessary for treatment planning and implementation.
Develops treatment plans, directs, supervises and treats patients in accordance with treatment plan using treatment modalities.
Prepares written report of diagnostic findings, intervention techniques used, progress made and follow-up status in a time frame according to policy and procedure.
Consults and communicates with patient, family and other multi disciplinary team members and contracted agencies.
