Amber Valley Short Term Assessment and Reablement Team are seeking a motivated, forward-thinking Occupational Therapist to join our dynamic Service. This is an exciting opportunity to work at the frontline of promoting independence, preventing hospital admission, and supporting timely, safe discharge home.

Our service provides rapid, person-centred assessment and reablement interventions for adults following a change in their health or functional ability. You will work collaboratively with colleagues across health, social care, voluntary sector partners, and most importantly individuals and their families to help people regain confidence and maximise their independence.

What You’ll Be Doing

• Completing holistic, strengths-based assessments within people’s homes or community settings.
• Developing goal-focused reablement plans that support functional improvement and reduce reliance on long-term services.
