Occupational Therapist

We are recruiting an Occupational Therapist to support our Discharge to Assess (D2A) / Reablement service, helping people regain independence at home following hospital discharge. You’ll work as part of a multi‑disciplinary team, completing timely assessments, setting practical goals, and supporting a “doing with, not doing for” approach that promotes confidence, safety, and measurable progress.

Key responsibilities

• Complete OT assessments in the community and contribute to discharge planning
• Develop and review reablement plans with clear, measurable goals
• Provide guidance to reablement/support staff on safe techniques and enablement approaches
• Recommend equipment and minor adaptations, and support onward referrals where needed
