Occupational Therapist Elizabethtown What you'll do Conduct comprehensive in-home assessments to evaluate patients' physical, cognitive, and functional abilities and establish individualized plans of care.
Develop and implement personalized treatment plans targeting activities of daily living (ADLs) and instrumental ADLs, with short- and long-term goals aligned to each patient's needs.
Assess the home environment and recommend modifications, adaptive equipment, and safety improvements to promote independence and reduce fall risk.
Deliver skilled therapeutic interventions focused on self-care, mobility, strength, cognition, and functional recovery — including for patients with complex neurologic diagnoses.
Educate patients, caregivers, and family members on therapeutic techniques, home exercise programs, and strategies to support ongoing independence.
