As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care , you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more-all working together to deliver personalized, high-quality care.

You will work onsite at our New Conviva Port Salerno Center in Stuart
Florida. You will report to our Associate Medical Director.
