Nurse Practitioner - Primary Care Liaison Service

About the role

The Primary Care Liaison Service offers a Nurse Practitioner (NP) position (or NP Candidacy) to enhance multidisciplinary care for people with mental health needs. The role focuses on clinical leadership, consultation with primary care and NGO sectors, and ensuring timely and community‑based care for tangata whaiora. For current Capital Coast and Hutt Valley Health NZ employees, the NP Candidacy period will be a secondment.

Salary: Senior Nursing Grade 6 as Nurse Practitioner (NP). For NP Candidacy the salary is Senior Nursing Grade 3.

Responsibilities

• Provide assessment, treatment, and transition support within secondary and primary mental health services.
• Collaborate with primary care providers to maximise treatment and support continuity of care.
• Provide clinical leadership and consultation to meet identified health priorities.
