Position Summary:

The Nurse Practitioner (NP) at Sweetgrass Health Centre will provide comprehensive primary health care to clients across the lifespan. Emphasizing health promotion, disease prevention, and chronic disease management, the NP will work collaboratively within a multidisciplinary team to provide culturally safe, holistic care. This includes the integration of both traditional and Western approaches, ensuring services are responsive to the needs of Indigenous individuals and families within the service area.

This role includes providing care to urban Aboriginal populations in Cochrane, Kapuskasing, and Hearst, as well as partner First Nations communities such as Taykwa Tagamou, ...
