Personalized Health Partners (PHP) Nurse Practitioners have a passion for delivering quality care to seniors and other residents in a variety of healthcare settings that can include, but are not limited to, skilled nursing facilities, assisted living, and independent living communities, as well as via telehealth.

PHP is currently recruiting a Nurse Practitioner with a passion for delivering the highest quality care for our residents at Riverside Healthcare Center located in Dayton, Ohio.

Our Nurse Practitioners work first shift, Monday - Friday. Never on-call and no weekends or holiday coverage.
