Nurse Navigator - Full time, Days - Cardiology - Atlantic Health Chilton Medical Center

+ Exceptional communication, organization and customer service skills.

+ The Nurse Navigator is responsible for enhancing the communication amongst the patient/family, the interdisciplinary team and all interdepartmental needs.

+ Assists in the development of strategies to reduce LOS and re-admissions.

+ Part of the cardiovascular team which monitors LOS and re-admission data for CHFAMI, COPD, and pneumonia.

+ Monitors care process and develops approach to improve care efficiency and patient outcomes.

+ Performs other related duties as assigned.

Schedule:

+ Full time, Days

+ 8am - 4pm

Required Education:


+ Bachelor's/Master's in Nursing or related field
License/Certification

+ Current NJ (New Jersey) Registered Nurse License required

+ BLS required
Relevant Experience

+ 3 years of cardiology nursing

+ Quality background preferred
