POSITION SUMMARY:

Makes nursing assessments and diagnoses. Works in conjunction with other members of the Interdisciplinary Team (IDT) to achieve positive health outcomes for persons receiving services.

+ Coordinates the delivery of routine and specialty health care services to address assessed needs of persons receiving services in assigned program unit.
