Nephrology Physician

Location: Brooklyn, NY

Job Type: Part Time - 14 hours per week

Facility: Out Patient Facility

Responsibilities

• Provide outpatient nephrology consultations and follow-up care
• Diagnose and manage chronic kidney disease and related renal conditions
• Develop individualized, evidence-based treatment plans
• Collaborate with primary care providers, nursing staff, and care coordination teams
• Ensure timely referrals and continuity of care
• Maintain accurate and timely medical documentation
• Participate in quality improvement initiatives
• Ensure compliance with federal, state, HRSA, and managed care guidelines

Qualifications

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Nephrology
