**Job Description**

Assesses and evaluates patients/families' social, psychosocial and environmental situations to determine patient needs. Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned

**Qualifications**

**_Requirements:_**

+ Masters in Social Work from an accredited school required. New York State License (LMSW) required.
+ Experienced with acute inpatient, experience discharges planning acute care. Knowledgeable SNF and home care referrals. Proficient in biopsychosocial assessments.
+ Knowledge of pertinent state and federal regulations regarding social work and discharge planning, familiarity with post-acute provider services, financial screening, and reporting requirements.
+ Minimum 1-2 years experience in an acute care hospital required.
+ Effective interpersonal, communication and organizational skills required, computer proficiency required. Ability to problem solves us...
