Our Family Health Center promises to be a busy place given new partnerships and collaborations. Be part of the team that is the region's leading provider of integrated primary and behavioral health care. Position is full-time and is in Bloomington, IL.

Health Center hours are Monday – Friday 8:00 a.m. to 5:00 p.m. and Thursday 8:00 am – 7:00 pm. FLEXIBLE SCHEDULING OPTIONS! Sign-on and retention bonuses available! Tuition reimbursement and loan repayment opportunities are also available.

Responsibilities

Provide high-quality, patient-centered, culturally sensitive mental and primary health care services. Assess patient needs and create care treatment plans. Manage patient care. Staff cases as appropriate. Provide direct medical prescriber services. Help develop programs.
