Memory Care Coordinator

+ Hours M-F 11a-7p
+ + Direct the day-to-day functions of the nursing assistants working in the memory care unit are in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

+ Ensure that all nursing personnel assigned to you comply with the written policiesand procedures established by this facility.

+ Participate and cooperate with any facility surveys made by authorized government agencies as requested by the Administrator or Director of Nursing.

+ Participate in implementation of the facility’s quality assurance program for thenursing department.

+ Assist with working on the floor as needed.

+ Be part of the on-call rotation

+ Oversee all memory support activities

+ Ensure that all residents residing in the memory care unit are engaged and receiving the care they need in order to live their best lives.

+ Complete and report audits.
