**EDUCATION & EXPERIENCE**

**Minimum Qualifications:**

+ Doctor of Pharmacy or Bachelor's degree in Pharmacy from an accredited school of pharmacy.
+ Three (3) years of related pharmacy experience OR Two (2) years of poison center experience a
+ One (1) year of supervisory experience.

**LICENSES, REGISTRATIONS, OR CERTIFICATIONS**

**Required:**

+ Licensed to practice pharmacy in the state of Texas or able to obtain Texas license within 90 days of hire.

**Preferred:**

+ Certified Specialist in Poison Information Certification
+ Diplomate of the American Board of Applied Toxicology
