Homecare with Heart is looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Starting rate for LPN: $26 per hour, RN: $30 per hour depending on experience. The LPN provides skilled visits in patients' homes. This position will be serving Trumbull County.

Responsibilities

What You'll Do:

• Administer medication in accordance with standards of practice.
• Encourage social rehabilitative skills and encourage self-care and independence.
• Perform client treatments in accordance with Plan of Care
• Assist the consumer in learning appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
