The position of Licensed Practical Nurse (LPN) is based in Care At Home Services. The LPN will work under the guidance of the Senior LPN Supervisor or LPN Case Manager to assess, plan, implement, and evaluate nursing care for assigned patients in accordance with competency guidelines and the standards of practice set by the College of Licensed Practical Nurses of BC (CLPNBC), current legislative scope of practice, and Care At Home Services policies and procedures.

Responsibilities

The LPN will perform the following duties: assess patient condition and needs, develop care plans, administer medications and treatments, monitor patient progress, document care activities, coordinate with interdisciplinary team, and ensure quality of care documentation in the electronic system.
