Job Summary:# # The LPN is responsible and accountable to the Patient Care Manager and/or Program Director. Demonstrates ability to take patient referrals, physician orders, and scheduling of patients.## Demonstrates the knowledge and skills necessary to provide care as appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to read data reflective of the patient#s status and interprets the appropriate information needed to identify each patient#s requirements relative to his/her age specific needs, and to provide the care needed as described in the department#s policies and procedures. #Has ability to act as member of the telehealth clinical team as assigned.
