Job Description
JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team! This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.
HOURS: Monday - Friday (3) 12-Hour Shifts 7AM-7PM. On-call every other weekend
TERRITORY : Crystal Lake, IL 60014
Salary: $26 - $38 hourly. Offer Based on Years of Experience

Qualifications:
- Graduate of an accredited practical nurse or a vocational nursing program
- Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
- Currently licensed as an LPN/LVN in the practicing state
- Must possess current CPR Certification
- Demonstrates good verbal and written communication, and organization skills
