Long-Term Care Best Practice Implementation Coach

Responsibilities

• Develop a regional plan to support the implementation of BPGs in the assigned Long-Term Care homes, outlining strategies to facilitate spread within the region and outreach to area long-term care homes.
• Seek, create and coordinate opportunities for promotion and integration of evidence-based best practices in the long-term care sector.
• Develop methodologies for promoting and supporting knowledge transfer, and act as a resource, where appropriate, for educators within LTC homes.
• Participate in and provide feedback to research and evaluation activities.
• Liaise with the RNAO LTC Best Practices Program Manager and the Associate Director to provide information for integration and coordination of activities across regions within the Long Term Care sector, including strategic planning, implementation, evaluation and resource development.
