Job Description

The LPN will provide mental health, substance abuse and outreach services to individuals who may have a mental health and/or substance use disorder. This role will work with community partners and local stakeholders to provide comprehensive care. As a part of the community-based or office-based team, the LPN will meet with individuals in the office and/or their own environment such as in the client's home, hospital, schools/work, etc. to assess any medical/mental health needs, administer medication, and check vitals.
