                Job Details:  
Position : Licensed Practical Nurse 
Location : 18 Frances St #3095, Woburn, MA 01801 
Facility : LTC/SNF 
Shift :  8s (Variable - Days/Nights) - five days in a week that includes EOW 
Contract : 13 weeks 
Start : ASAP

BLS/CPR  
Immunization records (PPD, Flu and Physical) if updated  
Covid Vaccination Card  
Requirements:  State LPN license 
BLS/CPR preferred 
 Min 1 -2 year of LPN experience in rehab or skilled nursing facility will be preferred . 
Duties include taking Vitals, assisting with ADL's, transportation, Medicine administration and reminders, Assisting RN's. 
Care/Rehab or Skilled Nursing Facility is preferred
