Key responsibilities include:

• Providing direct patient care in facility environments, including assisted living, rehab facilities, nursing homes, and adult day care
• Administering medications and treatments according to provider orders
• Monitoring, assessing, and documenting vital signs and patient status
• Collaborating with facility staff and promptly reporting changes in patient condition
• Supporting staffing relief coverage across assigned shifts and multiple facilities
