Title: Licensed Practical Nurse (LPN)

Location: Penn Yan, NY 14527
Term: contract 13 weeks'
Shift: 8hrs - All shifts (Five days in a week that includes every weekend)
Pay rate: $42/hr on W2
Pay Period: Weekly 
Start: ASAP  
REQUIRED: 
Must be licensed as a Practical Nurse. 
Min 1 -2 year of (LPN) experience in rehab or skilled nursing facility will be preferred . 
Must work every other weekend. 
RESPONSIBILITIES: Collects data relevant to resident's physical and psychosocial condition on an ongoing basis and communicates finding to RN and other members of the Interdisciplinary Team. 
Contributes to the creation of a developmentally age appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes involving the resident/fam...
